This year, there will be a change in policy regarding the medical release form. USA Volleyball and the Great Plains Region will no longer be requiring or collecting hard copy medical release forms. This decision was approved by our insurance carrier as we want to minimize the liability of physically collecting the forms.  Club programs can continue to collect the information, but it does not have to be provided to the Region or USAV.  Check with your club program if they require athletes to have a medical release form.
